Use Google Forms to Create Digital Restroom Logs

Teachers, 

If you are like me, you may have a difficult time keeping track of all of the passes or agenda-mates that you have signed for you students to go to the restroom. I wanted to create a way to have a digital running log of when my students are in the RR, and also how frequently they are going. Many of my students repeatedly ask to use the RR only during certain activities, which indicates they are trying to avoid certain subjects/activities. Please make a copy of my form below and edit it to meet your needs! Google Sheets will keep a running log for you with an automatic time stamp.
